MI Camps Fun Funds Member Enrollment Forms
If you need printed enrollment forms in addition to the online enrollment form, please submit a request to fundraising@gfs.com.
Please include the following:
• Enter “AOGcampership forms” in the subject line.
• Provide your name and phone number.
• Provide your requested number of forms.
The Fun Funds Member Enrollment Forms will be shipped to your local GFS Marketplace store. You will receive a confirmation that your request was received and it will inform you when and where your forms will be arriving.
